Me and My Home
A guide to making small changes, to help you live as well as you can, in the place you call home.
This guide has been designed to help older people and those living with dementia think about the place they call home. This information will also benefit anyone who would like to live more comfortably in their home as changes with time may affect how we live our lives.
A checklist has been produced for each area of the home. You can use the questions and tips to focus your attention on changes that could be made. Many of the tips are meant to be lifestyle changes and low cost but they can have a high impact on you being able to do things for yourself.
A5 spiral bound 24 page guide.
This guide was created by Charlotte Sutcliffe (charlotte.sutcliffe3@nhs.net), an Occupational Therapist with a focus on frailty and dementia, in collaboration with Professor Claire Craig and designer Helen Fisher from Designed with Care.
